Clay County Citizens with special health needs
can provide important updated information to
emergency personnel who come to your aid!
STEP 1
Please complete the Clay County Communication Center Data Sheet
and return to:
PROJECT ALERT
Box 192
Spencer, IA 51301
STEP 2
Please complete the Emergency Information Form and store it in
the PROJECT ALERT vial in your refrigerator's freezer door.
STEP 3
Display the PROJECT ALERT refrigerator Magnet on your
freezer door. This
will alert emergency responders that your PROJECT ALERT
vial is in your freezer.
PROJECT ALERT can help YOU prepare yourself & your
family for the unexpected with suggestions from the hand-out "Make a Plan
for Emergencies".
STEP 4
Make your Plan for Emergencies and assemble your own
emergency supply kit.
